Dooor handle rod on a 91 box...

For the second time, the door rod going between the door handle and the door release has come off the door handle end... Last time I thought it was the plastic retainer had broken, letting it fall out of the door handle. But on the 91 boxes, at least the police version, there is a bend in the rod, to keep it on the door handle. But somehow, it keeps coming off... (the end looks like a squared off "Z" that threads into the door handle hole, to keep it on the handle.) Is it also supposed to have a plastic keeper on it? I for the life of me can't understand why this keeps coming off, since it is so damn hard to put it on there to begin with... (You literally have to remove the striker on the door to put it back on) Any help would be appreciated!

AFAIK there should be a plastic bushing in there, same as the bushing on the door lock motors if you do not have the twist lock retainer used with the simple L rod end.

So it should have one in the hole on the door handle itself, where the rod goes thru the hole?
